Albright LIFE is where seniors go to thrive so they can stay in their homes as long as possible. Participants have the opportunity to come during the day for social activities, therapy and nutritional meals to keep their bodies and minds healthy and active. LIFE is comprised of dedicated professionals, caregivers and an on-site physician who provide a high level of all-inclusive onsite expert care as participants need it. Funding may be provided by Medicare, Medicaid, Long Term Care Insurance or private resources and participants must live in the service area of the LIFE Center, be 55+ and nursing home eligible. Alden Estates of Countryside Home
Countryside Home is a nonprofit, more than 100 beds skilled nursing care facility that is a part of Jefferson County in Wisconsin. The facility s campus features an atrium, work activity center and computer rooms. It includes a rehabilitation gymnasium that offers inpatient and outpatient services. Countryside Home also provides mental health, pharmacy and social services. The facility s each household includes a kitchen, dining room and outdoor courtyard. It offers physical, occupational, speech, respiratory and activity therapies. In addition, the facility provides restorative nursing and pain management services. Countryside Home offers dental, podiatric, psychiatric, optometric, wound care and urinary incontinence solutions.
