Arlington Hills Care and Rehabilitation Center provides full rehabilitation and skilled nursing services 24 hours, seven days a week and is located in Salt Lake City. Its on-site staff provides full physical, occupational and speech therapies services. It personalized therapy treatments are physician directed, reassessed and individual oriented. Its social and family services include family counseling, discharge planning and home care coordination, funding application assistance and personal funds management. It offers a wide variety of recreational activities to satisfy the daily physical and mental needs of residents. It accepts major insurance carriers, including Medicare, Medicaid, hospice, workers' compensation and other contracted insurance companies. Its medical and nursing staff includes medical director, nurse practitioner, registered and licensed practical nurses, certified nursing assistants, pharmacist, social worker, activities director, and physical, occupational and speech therapists.
