Family Heritage Care Center is a 50 bed Skilled Nursing Facility with highly qualified, caring staff. Our small size enhances our ability to provide more individualized, person-directed care. We understand that persons coming to stay with us require more than nursing care and offer a variety of services to meet each individual's spiritual, emotional and social needs. Located in Black River Falls, the heart of Jackson County, Family Heritage Care Center provides exceptional care with compassion and respect for each of its residents. Family Heritage Care Center is owned and operated by Heyde Health System, a multi-facility provider of long-term health care in West Central Wisconsin. Heyde Health System's corporate team assures that its facilities provides incomparable care to each of its residents.
