Family Hospice

Family Hospice provides support through a caring and highly trained team of professionals. These are individuals who combine a deep commitment to their profession with a generous dose of compassion. The hospice interdisciplinary team for each patient includes their physician and/or our medical director, a nurse, certified nurses aide, a social worker, and a chaplain. The team develops an individualized plan of care with the patient and their family. We encourage active family participation in any decision made in the care provided. Volunteers are included to provide companionship, emotional support, and respite care as needed. Home Care and Palliative Care. Medicare / Medicaid Certified and approved. Calls answered 24-7.
