Established in 1889 and incorporated in 1904, the Visiting Nurse Association provides home care, hospice at home and wellness services across a 14 county area from Boulder, south to Pueblo, Colorado. Medical and non-medical home care services are available on an intermittent basis 24 hours a day, 7 days a week. As Colorado’s oldest and largest home health care agency, the Visiting Nurse Association (VNA) has a long tradition of providing skilled care from compassionate, highly qualified professionals and dedicated volunteers. The VNA, a nonprofit agency, was founded by community volunteers in 1889 as the Flower Mission to comfort Denver’s sick poor.

Founded in 1993, Hospice & Palliative Care of Western Colorado provides comprehensive hospice care, palliative care and grief-related services. It is a community-based organization that employs more than 200 individuals and serves over 200 patients every day. Located in Grand Junction, Colo., the organization offers various programs and services to assist patients, their families and the community. It provides services in patients’ homes, hospitals, assisted living centers and nursing homes. The organization s team of professionals includes nurses, physicians, social workers, counselors and chaplains. It is a Medicare- and Medicaid-certified organization that offers respite care, pain control and individualized care plans. The organization’s palliative care division provides symptom management to patients suffering from life-limiting diseases.
