Frontier Home Health
Established in 2004, Frontier Hospice provides medical care services to more than 300 families in Pueblo, Fremont and El Paso counties in Colorado. The organization provides physician-supervised care according to an individualized plan for each patient. Frontier Hospice serves patients in a variety of settings, including private residents and long-term and assisted care centers. It serves terminally ill patients and their family members. Frontier Hospice pays for medications associated with terminal diagnosis and equipment, such as oxygen cylinders, beds and walkers.
