Heritage Park Care Center
Heritage Park Care Center located in Carbondale Colorado is proud to offer a continuum of care including post-acute skilled nursing, short-term physical, occupational and speech therapies as well as outpatient services. Rehabilitation is a focus at this facility which includes a 3000 square foot therapy gym, therapy pool and state-of-the-art equipment. Heritage Park uses an inter-disciplinary team approach in establishing a plan of care with the goal of returning to home and community life. Generations At Lowry
Admission Assessment Once you have chosen Generations at Lowry to be your new home, an on-site assessment will be completed with you to ensure that you meet the admission criteria and to determine your required level of care. Our criteria for admission are available upon request. The Admission Process After the assessment is completed, the next step is to have your primary care physician complete our physician’s assessment form. Because this form must be completed prior to admission, we are happy to assist you in communicating with your physician as needed. Once the physician’s assessment is complete, we will schedule a move-in date and provide you with our move-in packet to help you get acquainted with your new home. On or before the day of admission, we ask that you plan time to go over the admission agreement with our team. Admission and Continued Stay Criteria The criteria for admission and continued stay at Generations at Lowry will be determined by your current needs for services. Every resident of Generations at Lowry is continually assessed to ensure that we are meeting and can continue to meet their needs. Services and Levels of Care Independent Living A resident who fits into the Independent Living category is one who is functionally and socially independent. In addition to the apartment, monthly rent includes three meals per day, recreational activities within and outside the facility, weekly housekeeping & transportation.
Namaste Alzheimer Center
Namaste Alzheimer Center is a retirement community that offers assisted living and Alzheimer s disease care services for senior citizens throughout Colorado. It is designed, staffed and operated to serve people who suffer from various stages of Alzheimer s disease. Namaste Alzheimer Center provides meals, medical monitoring and nursing care services. It also offers residential care and assistance with personal care activities for its residents. The community is certified by Medicaid and provides Namast Adult Day Program, which focuses on ensuring maximum independence and developing social and medical status improvement plans. Namaste Alzheimer Center is a part of Centura Health, which manages several senior living communities throughout Colorado.
