Pathways Home Health, Hospice & Private Duty is a community-based, nonprofit health care organization with a location in Oakland, Calif. The organization offers home health, nursing and hospice care services. It also provides grief support and nonmedical attendant and elder care services. The center offers hospice care to children suffering from life-threatening illnesses. Founded in 1977, it provides palliative care, adult home infusion therapies, heart and lung disease management and diabetic educator services. The center offers rehabilitation services for strokes, fractures and traumas, and it also offers speech and language intervention services. In addition, it is a Medicare- and Medicaid-certified firm that is approved by the Joint Commission on Accreditation of Healthcare Organizations.
