Patients may receive hospice care in a variety of locations, including their home, or a contracted health care facility. Our team of physicians, nurses, social workers, hospice aides, spiritual care services, volunteers and bereavement coordinator are available around the clock to provide the best quality of care. Our staff is focused on comfort care, control of pain, and symptom management for the patient, while providing support to the patient’s family and caregivers through the assistance of ancillary team members. Evercare Hospice

At Evercare Hospice & Palliative Care, we are committed to providing the best hospice and palliative care experience available. It can also be measured by the services we offer, which go above and beyond what typical hospices offer. Unlike many hospice programs, Evercare provides the following services: We offer board-certified hospice and palliative care physicians at each of our program locations. Our medical directors regularly see our patients and are available for direct patient care. We have an unrestricted access program, even for those eligible patients pursuing aggressive treatments like radiation or chemotherapy. Because of our physician involvement, we provide a broad spectrum of medications and treatments for symptom relief.

Hospice By The Bay

The non-profit Hospice By The Bay serves Marin, San Francisco, Northern San Mateo and Sonoma counties, providing the highest quality of end-of-life hospice and palliative care to patients and support for their families. Its community Grief Counseling Program also comforts anyone coping with the loss of a loved one. A 501(c)(3) non-profit organization, Hospice By The Bay has served Bay Area families since 1975. Accredited by the Community Health Accreditation Program (CHAP) and certified by the Centers for Medicare and Medicaid Services, Hospice By The Bay care is funded through health care reimbursement from Medicare, Medi-Cal and private insurance, as well as charitable contributions from individuals and corporate donors.
