Lakeport Skilled Nursing Center provides nursing care services to its residents. It offers a wide range of services, such as discharge planning, health evaluation, and dietary, vision, podiatry and 24-hour skilled nursing care services. The services provided by the center also include diabetic monitoring, intravenous therapy, pain and wound management, transportation, financial assistance, and incontinence, respite, hospice and palliative care services. The center also offers a variety of private rooms with attached kitchens, bathrooms, and dining and living rooms. The living facilities provide by the center feature cable televisions and telephones. The rehabilitation services provided by the organization include physical, occupational and speech therapies. It also offers various facilities, including a beauty shop, barbershop and outdoor patios. The center is a part of the Horizon West HealthCare and is located in Lakeport, Calif.
