Community Bridges Center for Hope

Established in 1982, Community Bridges is a nonprofit organization that offers substance abuse treatment and recovery services for homeless and working poor adults in Maricopa County, Ariz. It also provides a range of prevention, education and training services in schools, public and private hospitals, police and fire departments and judicial establishments. The organization additionally offers medical detoxification services. Community Bridges has five operating locations with more than 130 employees, including doctors, nurses, clinical support staff, and administrative, operations and education personnel. It provides outpatient treatment and criminal justice services. The organization s services are licensed by the Arizona Department of Health, Division of Behavioral Health Licensure and National Commission on Accreditation of Rehabilitation Facilities. Community Bridges is located in Mesa, Ariz.

Health Exchange of Arizona has been an experienced home care provider since 1991 and prides itself on a reputation of providing outstanding caregivers and excellent customer service. Our experienced, professional staff interfaces with physicians and other care providers to insure a comfortable, secure and beneficial home care experience for every patient. Our lofty reputation is based on carefully screened nurses (RNs, LPNs, CNAs, HHAs, and live-in caregivers) who pass a series of employment screenings, including drug tests, criminal background checks and clinical/written examinations of skills. Only after passing all these tests and having a current license or certification, two business and personal references, at least one year of home care experience and current First-Aid and CPR licenses can our staff be assigned to your home. Then, your in-home health care can begin with a live-in, companion, or an attendant caregiver. There are many options you can choose from – but only one Health Exchange of Arizona.
