The center provides physical, speech and occupational therapies. It offers individualized nutritional evaluation and discharge planning programs. The Evergreen Health and Rehabilitation Center of Petaluma also provides hospice and social services. The center manages a dementia care unit and family support network. In addition, it offers individualized and group activity programs. The center accepts insurance plans from several companies, such as Medicare and Medicaid. The Evergreen Health and Rehabilitation Center of Petaluma is managed by EHC Management.
