Heartwood Hospice
Hospice is a fancy word for keeping people comfortable when they have a life-limiting illness. Our caregivers will visit your home and help keep you comfortable during this difficult time. Comfort care is meant to help you make the most of the last months of life by giving you comfort and relief from pain. The focus is on care, not cure. How does it Work? Your doctor and the Heartwood team will work with you and your family to set up a plan of care that meets your needs. Your primary caregiver may be a family member or other person who cares for you every day. We don?t replace your caregiver but instead are there for support and as a resource twenty four hours each day. “”””I will never forget the kindness and the gentle way you helped me and my family through this…”””” Sally, wife and caregiver, SLC. “”””Dear Heartwood… The love, care and concern you gave our family was especially appreciated.”””” Darlene, wife and caregiver, West Valley. “”””Heartwood was so compassionate and helpful. We could not have managed without them — they saved us.”””” Jan, wife and caregiver, South Jordan. Why choose Heartwood? We are a dedicated team of professionals that are absolutely passionate about the hospice benefit. If you will allow us, we will take care of you in your home just like you were family. Heartwood Hospice is Medicare and Medicaid certified. We are licensed by the state of Utah as an out-patient hospice. Which means that we can care for you in your home.
