Franklin County Home Health
Established in 1969, Franklin County Home Health Agency (FCHHA) provides home health, hospice, rehabilitation therapy and support services to people of all ages in Franklin County, VT. FCHHA is certified by the Centers for Medicare and Medicaid Services to provide Medicare and Medicaid services. Each year, we provide care to over 1,700 people in our region through our programs and services that include: home care, hospice and palliative care, long term care, choices for care, chronic disease management, physical therapy, occupational therapy, speech language pathology, high-tech nursing, maternal child health, Nurse-Family Partnership, public flu and foot clinics, as well as free Grief and Bereavement and Baby Bumps support groups. We reach out to first time, low-income pregnant women through our Nurse-Family Partnerhip program, covering Franklin, Lamoille and Grand Isle counties in Vermont. This program provides nurse home visits to participants to support them from pregnancy through their child’s second birthday. We extend our nurse home visits for pregnant and new mothers through our work with Vermont’s Children and Infant Services program. We hire compassionate, highly qualified professionals to provide our care. Our staff includes: Registered Nurses, Licensed Practical Nurses, Pediatric and adult physical and occupational therapists, pediatric and adult speech language pathologists, office personnel, hospice volunteers, Licensed Nursing Assistants and Care Attendants.
