The Evergreen Independence Health and Rehabilitation Center is a 70-plus-bed facility that meets short- and long-term care needs of residents. It offers a range of health care services and is located in Independence, Ore. The center also participates in Medicare and Medicaid programs. The Evergreen Independence Health and Rehabilitation Center is owned and operated by EHC Management, which offers consulting services to more than 55 licensed skilled nursing and assisted living facilities in Arizona, California, Idaho, Montana, Nevada, Oregon, Utah and Washington.
