Fall Creek Valley Care Center is a 60-bed skilled nursing care facility located in Fall Creek, Wis. Established in 1968, it features private and semiprivate rooms, dining areas, lounges, family room and a chapel with a rotating area clergy roster. It has separate private rooms for long-term residents and a specialized rehabilitation wing with private rooms, cable television and telephones. It has 24-hour licensed nursing staff, complete security, in-house personal laundry service, customized meals, cable television, salon services, spacious grounds, interior courtyard and attached gazebo. It offers various activities, including opportunities for interaction, socialization and exercising.
