Founded in 1995, Angel Foundation is an organization that raises funds from the recipient s home community to fulfill the needs of terminally ill children. Located in Van Wert, Ohio, the foundation is a part of Community Health Professionals, a home health care service provider. The holding firm s staff includes nurses, therapists, social workers, hospice specialists and home health aides. It is approved by the Community Health Accreditation Program and has the Ohio Bureau of Workers Compensation Certification. Community Health Professionals offers infusion therapy, diabetes education, and wound and ventilator care. The holding firm is a member of the National Association for Home Care, the Ohio Council for Home Care and the Visiting Nurses Association of America.
