PEP’s Mission Statement: The mission of Beth Abraham Health Services’ PEP For Seniors NORC program is to enhance the quality of life and independence of its diverse community residents. The goals of the program are to 1) enhance, extend and coordinate services available to seniors living in Parkchester so that they can successfully age in place; 2) to empower seniors to develop and implement programs that will benefit them, and 3) work collaboratively with the Parkchester community, NORC partners and funding agencies to coordinate quality programs and services.

Often, recovery after hospitalization requires inpatient care. At Wesley Rehabilitation Center, patients receive the short-term rehabilitation services they need in a comfortable, home-like setting. Whether recovering from a stroke, surgery, fractures, joint replacement, or recent illness, Wesley’s highly trained staff and individual care will help to achieve maximum independence and a quality of life that everyone deserves. Pre-application for scheduled orthopedic and medical surgeries are also accepted.

The Avalon is proud to offer the best way to â??age-in-placeâ?? via a specially designed, state approved approach known as the Assisted Living Program (ALP). That means (with few exceptions) as your personal care needs grow, the Avalon adjusts its services to meet those needs. And, for Medicaid qualified individuals, when private funds run out your housing and services at the Avalon are 100% covered by your benefits.

Your second line of help: If you need more than assistance on the phone, an Eldersource care manager will meet with you and or your family members. A care manager is a social worker who is trained in geriatrics AND the ins and outs of our community’s complex eldercare system.
