Lifetime Care
Established in 1960, Lifetime Care is a nonprofit organization that provides health care services for individuals of all ages. It offers treatment solutions for patients with diabetes, cancer, high blood pressure, Parkinson’s disease and heart problems. The organization provides wound care, home infusion, oxygen administration, blood testing, and pain and medication management services. It also offers physical, occupational, respiratory, massage and speech therapies. Lifetime Care additionally provides acupuncture, family counseling, and hospice and respite care services. The organization offers training, consulting and 24-hour emergency solutions, as well as bereavement programs. It is a subsidiary of Lifetime Healthcare Companies, which provides health insurance and care services to more than 175,000 individuals annually throughout the United States. The organization operates the Lifetime Care Hildebrandt center, which is spread across an area of over 9,600 square feet. Lifetime Care serves the residents of Rochester, Monroe, Wayne, Seneca and Livingston counties in New York.
Community Hospice
Based in Rensselaer, N.Y., Community Hospice serves individuals with chronic diseases and their families. It employs trained doctors, nurses, social workers, chaplains and volunteers who provide hospice care services to patients in their homes, hospitals and nursing homes. The center provides a variety of services for patients, including palliative care, physician education programs and bereavement counseling. Its range of services includes nursing, medical consultation, spiritual care, physical and occupational therapies, nutritional therapy, home health aides, medication management, respite care and adult day care. Community Hospice also provides a variety of other services, such as stress management, assistance with personal hygiene management, skin care and pain management. It provides referrals to community resources, including individual counseling services.
