Established in 1897, Visiting Nurse Association of Manchester & Southern New Hampshire is a nonprofit charitable organization that provides home health and hospice services. It offers home care services to help patients recover from surgery, physical disabilities, and short-term, chronic and life-limiting illnesses. The organization provides various community programs, including child care, parent education, and bereavement support and counseling. The organization also offers home care, homemaker, child health, hospice, personal and long-term care management services. It additionally operates the Visiting Nurse Association of Southern New Hampshire Foundation, which raises money for home health, hospice and community service programs. Visiting Nurse Association of Manchester & Southern New Hampshire is affiliated with the Visiting Nurse Association of America and the National Association for Home Care. Central NH VNA & Hospice

Community Health & Hospice was founded in 1975 as a visiting nurse organization, known as Lakes Region Community Health Agency. Located in Laconia, N.H., Community Health & Hospice is a private, nonprofit home health and community support service agency serving the residents of New Hampshire. The organization supports and educates families through pediatric visiting nurses, mother-baby postpartum visits, pediatric care management, prenatal home visits, immunizations, child development programs and partners in health. Its young family program serves children suffering from serious illness or developmental disability, premature infants, children with no health care provider and parents with concerns about child development. The organization also provides hospice care to terminally ill patients and bereavement counseling for family members and caregivers. Its hospice team includes the attending physician, medical director, registered nurses, licensed nursing assistants, therapists, nutritionists, medical social workers, counselors and volunteers.
