CenterWell Home Health

We are a family of home health, palliative, hospice, and community care providers. We are committed to clinical excellence and providing compassionate care and extraordinary service to the patients and families we serve. Our care coordination ensures patients receive the clinical care and the support they need while remaining at home, reducing hospital readmissions, and providing support for their caregivers. Our care matters. The right level of care, at the right time.

Specialized Home Care

Specialized Home Care was recognized and certified in 2014 by Centers for Medicare & Medicaid Services (CMS) as one of modern home-health care agencies which are scientifically measured and assessed to have high-quality home-health care services for promoting health and improving the quality of life. Specialized Home Care services include nursing, physical therapy, occupational therapy, respiratory therapy, speech pathology, medical social, and home health aides. Our team of specialists also include skilled nursing professionals, medical social workers and home health aides so whether you are recently injured or dealing with a chronic condition, we will provide you with your own, specialized plan for optimal recovery.

Kindred Hospice

We help people receive the highest-quality, most compassionate end-of-life care while surrounded by friends and family. Hospice care can take place in a variety of settings, wherever your loved one calls home. Our compassionate care provides the physical, emotional and spiritual resources needed to make the most of every moment, with the extraordinary service you expect. We understand your needs and goals, and support both in each step of your journey. Hospice care may be right for your loved one when living with an end-stage illness, which has a six-month life expectancy, should the disease run its normal course. There are no limits on the amount of time you can receive hospice, but your physician must order the service once every six months to determine if our care is still appropriate. We will be glad to complete an assessment to help determine if hospice is a treatment option for your loved one.
