Our HomeWell Care Services team provides quality in-home care from our local office in Edina, MN. We are committed to helping people live life more fully, from being a helpful companion to those who struggle with regular activities of daily living—such as grooming, cooking, errands, and medication reminders—to assisting with some of life’s biggest challenges, like chronic conditions, palliative and hospice care support. Gundersen St. Elizabeth’s Care Center
Gundersen St. Elizabeth’s Care Center Shields Ave. is an 80-bed skilled nursing facility near our main campus. It includes a specialized Alzheimer’s Unit for residents with moderate to severe dementia. Our compassionate caregivers provide holistic, person-centered care for residents that require a short-term rehabilitative stay or need long-term care. Together, we create an individualized care plan that includes medical and therapeutic care, meaningful activities, social connections and family engagement.
