Little Hospice

Established in 1996, N.C. Little Memorial Hospice is a nonprofit organization that provides pain management, symptom control and care services for individuals with a terminal illness. It serves patients with cancer, HIV/AIDS, stroke, amyotrophic lateral sclerosis, dementia, cardiovascular disorders, and liver and lung diseases. N.C. Little Memorial Hospice offers chaplain, social work, and hair and nail care, as well as 24-hour nursing care and laundry services. The organization also provides food services to patients. It is licensed by the Minnesota Department of Health and is a member of the National Hospice and Palliative Care Association and Hospice Minnesota. N.C. Little Memorial Hospice is located in Edina, Minn.

What if the focus during life’s final months was on living instead of dying? What if end-of-life care was as much for the family as it is for the patient? What if experts caring for you during end of life seemed more like family? Every hospice patient and family has a dedicated hospice care team. This helps to build relationships between the patient, family and hospice team. The hospice team works with the primary care provider, patient and family to set up a plan of care, which honors the patient’s goals, values and wishes. This plan of care describes who will visit, how often they will visit and what they will do. The plan of care will also determine the need for medicines, medical equipment and whatever else may be needed.
