Person-Centered Care Coordination Care Alliance of Western Massachusetts (CAWM) is a partnership of eight agencies working together to connect consumers with long-term services and supports. We are a team of compassionate professionals providing person-centered care coordination services for MassHealth ACO and MCO members ages 3-64. Our partner agencies have over 4 decades of experience in Hampden, Hampshire, Berkshire, Franklin, and Worcester Counties.

Adelaide of Newton Centre – Memory Care

Adelaide of Newton Centre is a proud to be a memory care community within the Benchmark family. With over twenty years of experience, Benchmark is able to use years of hands-on knowledge that fuels our compassion for our residents. We’ve created a culture of human connection—that connects residents to the people, passions and things that matter to them. At Adelaide, our goal is to bring that experience, engagement and empathy to our residents and their families – providing first of its kind comprehensive brain programs to older adults suffering from early onset Alzheimer’s, varies forms of Alzheimer’s and forms of dementia. Adelaide welcomes residents, family members, and community members to experience the benefits of innovative technologies and programs designed to stimulate brain activity for our residents. Our staff is not only focused on stimulating the brain but are also looking at the whole picture– overall wellness, nutrition and fitness needs.

Blue Hill Adult Day Health Center

Blue Hill Adult Day Health Center was created to improve the quality of life for those in need of specialized care and to provide support for their families. We aim to provide a compassionate and accommodating environment with a professional staff to handle our client’s needs. Offering a variety of medical, nursing, rehabilitative, social and recreational programs is at the heart of our philosophy.

Senior Helpers

Our mission is to ensure a better quality of life for our elderly clients and their families by providing dependable and affordable care. We are a trusted and respected organization that takes a responsible role in supporting our clients and their families, our employees, and our community. For our basic level of care, caregivers provide services such as companionship, meal planning and preparation, light housekeeping, laundry, medication reminders, and other general assistance. For sitter services, caregivers sit with and monitor a client during their hospital or facility stay. The caregiver can then follow the client home to provide a smooth transition.

The Willows at Westborough

The Willows at Westborough is an independent senior living facility that has been operational for more than 20 years and offers a wide range of care services. Its services include weekly housekeeping, apartment maintenance, personal care, full-time registered nursing, educational classes, wellness and blood pressure checkups, dining, banking, dry cleaning, and shuttle and snow removal services. The organization also provides various facilities, such a as 24-hour emergency response system, community room, computer laboratory with Internet access, arts and crafts studio, woodworking shop, music, game and billiard lounges, a beauty salon, barbershop, general store and library. The Willows at Westborough is a member of the Massachusetts Extended Care Federation. It is a part of the Salmon Family of Services that owns and operates a group of companies and provides senior living services. The company offers a wide range of residential, health care and management consulting services in Natick, Northborough, Northbridge and Westborough, Mass.
