Project Independence, a community-based, non-profit organization, was founded in July 1982 by a group of individuals concerned with the health and well-being of the elderly and disabled who needed assistance and support in order to remain independent and live at home. Our adult day health center provides daily medical and social activities to prevent and postpone nursing home placement. Braemoor Health Center
Based in Brockton, Mass., Braemoor Rehabilitation and Nursing Center is a 120 Bed, skilled care facility that provides health care services. The health care center has been operational since 1939, and offers a range of services, such as pain management, intravenous therapy, total parental nutrition, oxygen therapy, post cardiac monitoring, wound care, stroke rehabilitation and care planning, among others. In addition, it provides hospice care, short term rehabilitation, respite care, general evaluation and observation services prior to hospitalization.
