The environment, located just minutes from Monument Health Custer Hospital and Clinic, offers 16 private rooms equipped with private bathrooms. All residents are served three well-balanced meals a day as well as snacks, all of which are prepared daily in our kitchen.Monument Health Assisted Living is licensed to provide care for residents who are cognitively or physically impaired, dependent on supplemental oxygen, require a special diet, and/or need medication administration or management. Monument Health Home+ Home Health and Hospice House
Providers at Monument Health Home+ Home Health and Hospice House offer end-of-life care to patients in a comfortable, homelike setting. Skilled nurses and physicians manage pain and symptoms to improve the patients quality of life. Hospice House provides acute (24/7 in-home care), respite (temporary) and residential (patient resides at Hospice House) care. Monument Health understands that decisions about hospice care can be very difficult. Please talk with your primary care physician if you have questions about yourself or a loved one entering hospice care. Patients and families at Hospice House have access to pastoral, financial, grief and family counseling. Hospice is on-call 24 hours a day, 365 days a year.
