The Avalon Care Center Shadow Mountain is a 120-plus-bed facility located in Scottsdale, Ariz. It is owned and operated by Avalon Health Care, which provides a range of hospice, rehabilitative, psychiatric and community-based services. Founded in 1989, Avalon Health Care offers medical services in Utah, Arizona, California, Washington and Hawaii. It provides advanced physical therapy and other rehabilitative services through an ancillary service company called Brighton Rehabilitation. The organization works in partnership with Valley Mental Health to deliver mental health care services to nursing home residents. Avalon Health Care s subsidiary, Bristol Hospice, addresses the physical, emotional and spiritual needs of individuals through licensed and certified hospice care programs.
