To that end the rehabilitation professionals at Cherry Brook are committed to helping you achieve your highest level of function in order to return home in the shortest time frame possible. Our highly trained and experienced therapists work with you to develop an individualized care plan to guide you toward successful and complete rehabilitation. Your care plan begins with pre or post hospitalization and progresses through your inpatient stay to discharge home. We will facilitate your transition home with home evaluation services or out-patient referrals.
