Hospice philosophies and goals of care differ from program to program. All hospices share a goal of patient and family centered care for those at the end of life. At CCHCT we do not believe that hospice is stopping all of your medications and giving you pain medication. We believe that by hiring experts with disease specific training and utilizing the latest research, we can predict most of the symptoms that may develop and prevent or treat those symptoms in your home. Hospice care delivered by the professionals at CCHCT can continue to improve and meet the needs of patients by partnering with hospitals, physicians, home health companies, and community members to allow patients to transition across the healthcare continuum without barriers. At CCHCT we strive to make it possible for you to have control over your disease so that a balance can be achieved between comfort and quality of life.
