Fairview Home Care & Hospice

Fairview Home Care and Hospice offers a range of rehabilitative and care management services. It focuses on the physical, spiritual and emotional needs of individuals. The center provides the treatment of various cardiac and neurological conditions, as well as multiple sclerosis, arthritis, stroke and diabetes. Fairview Home Care and Hospice offers nutrition consultation, medication monitoring, physical and occupational therapy, and speech and language pathology services. It provides a variety of equipment to help monitor blood pressure, heart rate, temperature and oxygen level. The center also offers 24-hour private-duty nursing services. It participates in Quality Partners, which is a national, cooperative effort developed and directed by the National Hospice and Palliative Care Organization (NHPCO). Fairview Home Care and Hospice is Medicare-certified and accepts the health care plans of several insurance companies. It is operated by Fairview Health Services, which is one of the largest, nonprofit health care systems in Minnesota.
