Heritage Park Care Center located in Carbondale Colorado is proud to offer a continuum of care including post-acute skilled nursing, short-term physical, occupational and speech therapies as well as outpatient services. Rehabilitation is a focus at this facility which includes a 3000 square foot therapy gym, therapy pool and state-of-the-art equipment. Heritage Park uses an inter-disciplinary team approach in establishing a plan of care with the goal of returning to home and community life.
