Partners in Senior Care provides in-home private duty care and care management services for senior citizens throughout the Chicagoland area, including Cook, Lake and McHenry counties in Illinois. It provides home care services that include light housekeeping, meal planning and preparation, and assistance with ambulation and exercises. The facility offers personalized care services that include assistance with daily living activities, including bathing, medication reminder and personal laundry. Partners in Senior Care administers the Geriatric Care Management and Personal Care program, which evaluate the client s physical, emotional, functional and social strengths and needs. The facility also provides rehabilitative and hospice care solutions. It is a member of the Illinois Continuity of Care Organization.
