Senior & Family Consultants

This is a Geriatric Care Management Service. Senior and Family Consultants assists seniors with obtaining needed resources to remain in their home safely. We assist seniors and their caregivers in finding the resources in the community that they need. We complete a full assessment in their home and provide recommendations. We then monitor these services and advocate on behalf of the senior and their family.
