Founded in 1995, SouthernCare is one of the largest hospice providers in the United States. Headquartered in Birmingham, Ala., it is a chain of health care facilities that offer hospice care services for individuals with chronicle illnesses and disabilities. SouthernCare is a privately owned company that maintains more than 90 offices in over 10 U.S. states. With one of its locations in Montgomery, Ala., the company provides hospice services to patients who reside in private homes, group homes, and assisted living and skilled nursing facilities. It maintains a staff of skilled and professional registered nurse case managers, social workers, home health aides and chaplains. In addition, it also operates a network of volunteers that offers support services to its staff.
