Community Care Centers is a nursing home management company that provides management services to 14 nursing homes and seven residential care facilities. Its skilled nursing facilities offers a wide range of services, such as hospice care, injectable medications, pain management, diabetic care, wound care management, postsurgical care, gastroenterology and in-home laboratory. It also offers rehabilitation therapy services that include physical, occupational, speech and restorative therapies. It has a care unit that provides specialized activities, including small group activities, music therapy, supervised outdoor activities, enhanced one-on-one interactions and field trips.
